The Chairman of the NRC IMC0350 Panel for the reactor head degradation
is pictured studying the FENOC presentation handout on the restart plan, at
the IMC0350
meeting of 7-16-02. He expressed "frustration" that the Davis Besse Root
Cause summary was
missing some essentials. FENOC is expected to publish the root cause to the NRC for
review around
August 1, 2002. The NRC cannot wait for this document on the Human Factors (Software)
issues Root
Cause. The root cause document is slow in coming because of a highly detailed root
cause analysis
being conducted by industry experts, using a process known as "MORT", an
acronym meaning
"Management Oversight and Risk Tree" patented by a company called "Conger
and Elsea",
which has sent some key representatives to the Station to ensure proper
implementation
of the process. This work is underway at the Davis-Besse Nuclear Power Station.
I will leave you to utilize the body language assessment technique in the above photograph
to determine the extent of NRC "Buy-In" or .... the profound shortage of
it.
